Initial Visit appointment 

• Patients must be 18 years of age or older.
• If an appointment is cancelled at least 24 hours before the scheduled appointment time, a full refund will be provided.
• If the appointment is not cancelled or is cancelled within 24 hours of the scheduled appointment time, no refund will be provided.
